Passport Information

Passport applications can be downloaded from the US Department of State web site.
Download the form and fill in all the information. Then, take it to a passport processing center.

Immunization Information

What immunizations do I need?

The CDC guidelines are the primary source for required immunizations. To find the current recommendations, click here. At the time of this posting, the recommendations are as follows:

  • Hepatitis A or immune globulin (IG).
  • Hepatitis B, if you might be exposed to blood (for example, health-care workers), stay >6 months in the region, or be exposed through medical treatment.
  • Rabies, if you might be exposed to wild or domestic animals through your work or recreation.
  • Typhoid, particularly if you are visiting developing countries in this region.
  • Yellow fever vaccination - required for visa.

As needed, booster doses for tetanus-diphtheria and measles. (CDC Web Site 2-1-08)

 

Where can I get the immunizations?

Immunizations can be given through your primary care provider or through your county health department. The provider should be able to verify what immunizations you will need and will be able to advise you regarding malaria prophylaxis.
